Please visit the secure Client Portal below to access the new client intake forms. It is important to complete these forms prior to your initial visit.

Upon accessing the portal, you will be asked to create a username and password. You will then be directed to the intake questionnaire and required consent forms. These forms can be read and signed electronically.

*Please note: The client portal is only available to new clients who have completed a phone consultation and have been scheduled for an appointment.
